The Phoenix Union High School District offers a generous and comprehensive benefits package to eligible employees and their dependents. The District provides Health (Middle Option), Mid-Term Disability and Life Insurance on employees at no cost. End of Employment: If you sever your employment with PUHSD, all insurance benefits via the PUHSD will terminate on the last day of the month in which your contracted employment ends with the exception of the flexible spending accounts which terminate the last day of your employment. You will be offered COBRA continuation coverage whereby you will be eligible to keep your medical, dental and/or vision insurance for an additional 18 months. You can also convert your life insurance to an individual policy; if interested in conversion, please contact the Benefits Department @ 602-764-1538. Mid-year Changes: Generally you will not be allowed to change your benefit elections or add/delete dependents mid-year unless you have a Special Enrollment Event or a Mid-year Change in Status Event. In the event, you experience a qualifying event, you must notify the plan within 31 days of the mid-year change in status event by contacting the District’s Employee Benefits Specialist at 602-764-1538. The Plan will determine if your change request is permitted and if so, will provide further instructions.

Arizona State Retirement System (ASRS)
Regular employees working 20 hours or more per week for more than 20 weeks must participate in the ASRS by contributing a specified percentage to the Plan. The retirement benefit an employee receives depends on the number of years and months for which the employee receives retirement credit. Individual Retirement Plans
All employees, including part-time and temporary employees, are eligible to establish payroll deductions for contributions into the District Sponsored 403(b) & 457(b) Plans. TSA Consulting Group, Inc (TSACG) is the administrator. TSACG is an independent provider of compliance administration services.
